[gentoo-amd64] Upgrade from 2004.3 profile to the 2005.0 profile

Hello,

I still have to upgrade one of our two webservers from the 2004.3 profile to 
the 2005.0 profile, but the upgrade guide sems to have disappeared. Can 
someone provide me with the necessary information? The upgrade was delayed 
because of serious problems with  PHP and OpenAFS during the upgrade of the 
other server which had  to be stable before considering to upgrade the 
remaining machine.
