I talked to Mr. Chew today, and we do, indeed, need to get
this paperwork filed by Saturday (otherwise we have to pay
a $200 fine, although I don't understand that since the paperwork
itself quotes a $10 late fee).  A legible copy of the form to be filed is

Note that in filing this report we will be proposing dates
