13 are very bad signs. Nine times out of ten it means the drive is dead,
14 or nearly so. The few times it's not a dead/dying drive, is usually due
15 to dma not being supported, or a bad cable. Given that dd is not
16 working, and nothing seems able to actually write to the drive, I'm
17 leaning towards a dead drive.

19 As a last ditch attempt, you could try reseating the power and ide
20 cable on the drive, as well as the motherboard end of the ide cable. If
21 you have another system laying around, you could put the problem drive
22 in there to confirm it is a drive problem.
