Do you have support built-into the kernel for this device:

03:00.0 SCSI storage controller: LSI Logic / Symbios Logic SAS1064ET PCI-Express Fusion-MPT SAS (rev 04) ??

Device-Drivers >> SCSI device support >> SCSI Transports >> SAS Transport Attributes *
Device-Drivers >> SCSI device support >> SCSI Transports >> SAS Domain Transport Attributes *

Device-Drivers >> SCSI device support >> SCSI low-level drivers >> LSI Logic New Generation RAID Device Drivers *
LSI Logic Management Module *
LSI Logic MegaRAID Driver *

Device-Drivers >> SCSI device support >> SCSI low-level drivers >> LSI Logic MegaRAID SAS RAID Module *

All these bits should be in there NOT as modules if you're booting off an array that's on one of those controllers.

If you cannot build it into a monolithic kernel.. then you'll have to make sure all this stuff gets into your initrd image.. which I can't help you with. I *ALWAYS* build all disk-access bits directly into the kernel.

I hope this helps!
