4 My bet is that you are also running qmail-scanner correct? I have had
5 this happen with qmail-scanner and basically what appeared to have been
6 the culprit is when qmail-scanner tried to inject the mail for
7 delivery... something failed. qmail-inject received the email but
8 qmail-scanner didn't realize it so it tagged for delivery again...
