I don't see what changing the slots has to do with it. If you only run 
db-4.1.x then run that. There is no need to to have db-4.0 in that case

Not package mask, each and every profile, e.g.:

/usr/portage/profiles/default-x86-1.4/packages

Don't mask sys-libs/db, either remove it from world, or inject db-4.0.14
