I could find only two instances of string prefix matching in the tree:

- autotools.eclass depends on "=sys-devel/autoconf-2.1*" (with
  WANT_AUTOCONF=2.1).
- media-video/nvidia-settings has "=x11-drivers/nvidia-drivers-3*".

Both should be are easily fixable, in a way that doesn't break
backwards compatibility with current Portage.

So I think that fixing the spec would be possible, indeed.
